Job Summary
Provide intensive linkage and retention outreach services to approximately 20 vulnerable patients living with HIV and experiencing homelessness or housing instability. Accompany clients to medical, social service, housing, and legal appointments, then debrief and update the HIV team. Conduct field-based outreach to locate patients at risk of disengagement, co-create Individual Service Plans, and monitor implementation in the electronic medical record. Offer motivational interviewing and risk reduction counseling for substance use disorders, advocate for patients in the criminal legal system, and coordinate discharge planning for inpatients. Complete all reporting duties, fulfill mandatory reporting requirements, and participate in weekly multidisciplinary meetings.
Required Qualifications
- Associate's, or bachelor's degree in a related field and/ or equivalent to two years' relevant human services experience
- Experience and interest in working with people living with HIV and experiencing homelessness and housing instability
- Experience conducting outreach in non-clinic-based settings
- Computer proficiency
- Strong organizational skills
- Strong interpersonal and communication skills
- Experience in behavioral de-escalation
- Experience with electronic health records
- Experience in trauma informed care, motivational interviewing, and risk reduction frameworks
- Bilingual English-Spanish
- Demonstrated interest in working with vulnerable populations including those with active substance use disorder and mental illness
- Interest and willingness to work in multidisciplinary team setting
- Commitment to equitable and culturally appropriate care for a wide range of diverse populations
- Successfully complete the MassHealth Certified Application Counselor exam (CAC) within 60 days of hire and maintain active certification status
- Proof of Covid-19 vaccination(s)
