Job Summary
Coordinate follow-up care plans for members with less complex needs by scheduling appointments or enrolling members in programs; assess compliance with medical treatment plans via telephone or on-site visits; identify barriers to plan adherence and coordinate resolutions; collaborate with case management and providers to modify treatment plans and determine need for additional services; identify and refer to local, state or federally funded programs; coach members on reducing health risks; prepare reports documenting case and compliance updates; establish relationships with relevant agencies and participate in program partnerships.
Required Qualifications
- H.S. diploma or equivalent and minimum of 1 year of related experience; or any combination of education and experience which would provide an equivalent background
