Job Summary
Develop and implement strategic outreach, patient navigation, and community engagement initiatives to improve access to care and reduce barriers for underserved populations. Lead and oversee Community Health Workers, Resource Navigators, and related programs while establishing annual goals, performance metrics, and quality improvement plans. Build collaborative relationships with healthcare organizations, tribal partners, and community stakeholders to strengthen partnerships and coordinate clinic-sponsored events. Monitor program effectiveness using data to identify growth opportunities and ensure compliance with HRSA requirements and FQHC regulations. Recruit, train, and mentor staff while fostering a culture of accountability and continuous improvement. Travel frequently within Clallam and Jefferson Counties to represent NOHN at health fairs, coalitions, and outreach events, supporting mobile health deployment and addressing social drivers of health.
Required Qualifications
- Bachelor's degree in Public Health, Healthcare Administration, Social Work, Human Services, Community Health, Business Administration, or a closely related field
- Master's degree in Healthcare Administration, Public Health, Business Administration, Social Work, or related field
- Minimum of five (5) years of progressively responsible experience in healthcare, public health, community health, social services, care coordination, population health, community outreach, or related field
- Minimum of three (3) years of supervisory or leadership experience
- Experience developing community partnerships and leading cross-functional initiatives
- Experience in an FQHC, Community Health Center, rural health setting, public health agency, or integrated healthcare environment
- Experience with social drivers of health programs, Community Health Workers, patient navigation, or population health programs
- Experience managing grants, community-based programs, or federally funded initiatives
- Strong knowledge of FQHC operations, HRSA requirements, and community health principles
- Understanding of social drivers of health and their impact on patient outcomes
- Knowledge of healthcare access challenges experienced by rural and underserved populations
- Excellent leadership, coaching, and team development skills
- Strong project management and program implementation abilities
- Ability to develop strategic partnerships and facilitate community collaboration
- Demonstrated ability to analyze data and utilize metrics to drive improvement
- Exceptional verbal, written, and presentation communication skills
- Excellent organizational, problem-solving, and decision-making abilities
- Proficiency with Microsoft Office applications and healthcare information systems
- Ability to manage multiple priorities in a fast-paced environment
- Reliable transportation and valid driver's license required
- Ability to travel between clinic sites and throughout the service area
- Occasional evening or weekend attendance at community events may be required
