Job Summary
Conduct outreach activities at schools, community events, and local businesses to educate families on affordable health insurance coverage through Covered CA, Medi-Cal, and the Children's Health Insurance Program. Perform in-reach phone calls to current patients for enrollment assistance and provide education on community resources like Cal Fresh and SDG&E CARE. Network to establish community partnerships, receive referrals from collaborating agencies, and perform inter-departmental referrals for medical, dental, and behavioral health services. Maintain complete data for monthly reporting, prepare quantitative and qualitative reports on outreach activities, and share patient feedback to improve healthcare access. Work Monday through Friday with availability for evening and weekend events to support community meetings.
Required Qualifications
- High school/GED
- Covered California certification
- Valid CA driver's license
- Proof of auto insurance
- Availability to work evenings and weekends as needed
- Ability to lift/carry 50 lbs/weight
- Ability to stand for long periods of time
- Follow all safety procedures as outlined in Neighborhood Healthcare's Illness and Injury Prevention Plan (IIPP)
- Maintain current knowledge of policies and procedures as they relate to safe work practices
- Follow all safety procedures and report unsafe conditions
- Use appropriate body mechanics to ensure an injury free environment
- Be familiar with location of nearest fire extinguisher and emergency exits
- Follow all infection control procedures including blood-borne pathogen protocols
- Maintain privacy of all patient, employee and volunteer information
- Comply with all regulations regarding corporate integrity and security obligations
- Report all behavior and/or activity that are unethical, fraudulent, or unlawful
Desired Qualifications
- One year of experience in government or community program eligibility/screening for Medi-Cal or Medi-Cal for families
- Certified Enrollment Counselor for Covered CA experience
- Bilingual (English/Spanish)
- Knowledgeable about and experience with health care and social service resources in the community
