Job Summary
Collect and analyze patient data pre, during, and post hemodialysis treatments to adjust treatment plans and document outcomes in medical records. Assess and educate patients and families on learning needs, treatment modalities, and transplant options. Administer medications per approved algorithms, ensure correct laboratory collection and shipping, and initiate emergency response measures as needed. Direct daily hemodialysis therapy delivery in collaboration with direct and ancillary staff, while delegating tasks to Patient Care Technicians, LVNs, and LPNs under supervisor guidance. Participate in staff training, orientation, and quality improvement activities to enhance patient care outcomes within the End Stage Kidney Disease team. Requires 9+ months RN experience and 3+ months in hemodialysis; successful CAP completion required for advancement.
Required Qualifications
- Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse
- Chronic/acute hemodialysis experience
- Must have a minimum of 9 months experience as a RN
- Must have a minimum of 3 months experience in chronic/acute hemodialysis as a RN
- Graduate of an accredited School of Nursing
- Current appropriate state licensure
- Current or successful completion of CPR BLS Certification
- Must meet the practice requirements in the state in which he or she is employed
- Must be able to lift 200 lbs.
- Must be able to lift chemical and water solutions of up to 30 lbs.
- Must be able to bend over
- Must be able to move machines and equipment of up to 200 lbs.
- Must be able to move patients and assisting with ambulation
- Must be able to stand for frequent, prolonged periods
- Must be able to work in a health care facility with moderate noise levels
- Must be able to be exposed to infectious and contagious diseases/materials
- Must be able to travel to training sites or other facilities
- Must be able to provide essential functions of this position in other locations
- Must be able to perform essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses
- Must be able to perform ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment
- Must be able to assess, collaborate, and document patient/family's basic learning needs
- Must be able to administer medications as prescribed or in accordance with approved algorithm(s)
- Must be able to initiate or assist with emergency response measures
- Must be able to serve as a resource for health care team
- Must be able to ensure correct laboratory collection, processing and shipping procedures are performed
- Must be able to identify expected outcomes, document and update the nursing assessment and plan of care
- Must be able to ensure patient awareness related to transplant and treatment modality options
- Must be able to complete CAP requirements to advance
- Must be able to delegate appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians
- Must be able to participate in decision-making, teaching, leadership functions, and quality improvement activities
- Must be able to perform all other duties as assigned by Supervisor
- Must be able to perform all essential functions in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations
