Job Summary

Conduct outpatient hemodialysis therapy for assigned patients, including pre- and post-treatment data collection, medication administration, and emergency response. Collaborate with direct care staff to assess, plan, and evaluate patient care while documenting outcomes and adjusting treatment plans as needed. Provide education to patients and families regarding transplant options and treatment modalities, and mentor new employees through precepting and coaching. Ensure accurate laboratory collection and shipping procedures are followed. Participate in quality improvement projects and interdisciplinary team activities to enhance patient care outcomes. Requires minimum of two years in nephrology nursing and three months in chronic/acute hemodialysis.

Required Qualifications

  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Must meet the practice requirements in the state in which he or she is employed
  • Must have a minimum of 2 years of Nephrology Nursing experience
  • Must be able to lift 30 lbs.
  • Must be able to lift up to 200 lbs.
  • Must be able to bend over
  • Must be able to stand for prolonged periods
  • Must be able to move machines and equipment
  • Must be able to move patients and assist with ambulation
  • Must be able to work in a moderate noise level environment
  • Must be able to work in an air temperature controlled environment
  • Must be able to be exposed to infectious and contagious diseases/materials
  • Must be able to travel to training sites or other facilities
  • Must be able to provide essential functions of this position in other locations
  • Must be able to perform essential functions with reasonable accommodations
  • Must be able to work in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations
  • Must be able to perform duties under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses
  • Must be able to perform ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment
  • Must be able to make adjustments or modifications to treatment plan
  • Must be able to notify Team Leader, Charge Nurse, Supervisor or Physician as needed
  • Must be able to assess, collaborate, and document patient/family's basic learning needs
  • Must be able to provide initial and ongoing education to patients and family
  • Must be able to direct and provide all aspects of the daily provision of safe and effective delivery of hemodialysis therapy
  • Must be able to administer medications as prescribed or in accordance with approved algorithm(s)
  • Must be able to document appropriate medical justification and effectiveness
  • Must be able to initiate or assist with emergency response measures
  • Must be able to serve as a resource, leader, coach, mentor and role model
  • Must be able to set examples of appropriate behavior, work habits and attitudes
  • Must be able to ensure correct laboratory collection, processing and shipping procedures are performed
  • Must be able to reschedule missed or insufficient laboratory collections
  • Must be able to identify expected outcomes, document, and update the nursing assessment and plan of care
  • Must be able to ensure patient awareness related to transplant and treatment modality options
  • Must be able to participate in education and quality improvement projects
  • Must be able to serve as a Preceptor to new employees
  • Must be able to complete CAP requirements to maintain or advance
  • Must be able to delegate appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs, and Patient Care Technicians
  • Must be able to participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Must be able to work in a drug-free workplace
  • Must be able to work in a health care facility
  • Must be able to interact with patients, facility/hospital staff and physicians
  • Must be able to work with assistance from Equipment aids and/or coworkers
  • Must be able to work with assistance from coworkers
