Job Summary
Evaluate, plan, implement, and document physical therapy treatment for patients based on physician orders. Develop individualized treatment plans with achievable functional goals and revise them as needed. Instruct and supervise Physical Therapist Assistants, while educating patients and caregivers on therapeutic home programs. Complete appropriate documentation and billing in a timely manner. Must hold a valid Physical Therapist license and CPR certification, obtainable within 90 days of employment. Requires ability to lift 50 pounds and assist with 100 pounds.
Required Qualifications
- Graduate of an accredited school of physical therapy
- Physical therapy degree
- Minimum of a Bachelor's degree (if graduated prior 1/1/2016)
- Doctorate of Physical Therapy (if graduated after 1/1/2016)
- Minimum of a Bachelor's degree in Physical Therapy (International Candidates)
- Doctorate of Physical Therapy degree preferred
- Physical Therapist license in the State where services will be provided
- Eligibility to receive a Physical Therapist license
- State licensure obtained within the first 90 days of employment
- Certification in CPR
- Eligibility to become certified in CPR within the first 90 days of employment
- Ability to maintain all certifications and license according to expiration date
- Valid driver's license (if assigned to drive a company vehicle)
- Proof of insurance (if assigned to drive a personal vehicle)
- Ability to lift 50 pounds
- Ability to assist with 100 pounds
- Ability to push up to 300 pounds
- Willingness to work at other locations as assigned
