Job Summary
Conduct outpatient home health visits to adult and geriatric clients in Independent and Assisted Living Communities throughout the Central Bucks area. Perform diagnostic tests measuring mobility, range of motion, strength, balance, and safety assessments. Develop and implement individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while collaborating with nursing on multidisciplinary plans. Educate clients and family members on rehabilitative care and activities promoting independent living. Accurately document observations and interventions using a state-of-the-art touch pad tablet. Requires a current PA Physical Therapist license, one year of recent experience, and graduation from an accredited program.
Required Qualifications
- A current PA Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills
