Job Summary
Outpatient Physical Therapist (PT) for part-time home health visits in senior living communities within York County. Responsibilities include making outpatient visits to clients in designated geographic territories, performing diagnostic assessments (mobility, range of motion, transfer status, ambulation, strength, balance, safety), developing and implementing individualized care plans with manual therapy, gait training, balance, and other interventions, continually reassessing and revising the care plan, educating clients and families, and accurately documenting observations and evaluations in EMR. Requires PA PT license, minimum of 1 year experience, graduation from an APTA/CAAHEA-approved program, ability to work independently, strong interpersonal/organizational/problem-solving skills, and EMR experience preferred. Salary range $50-55 per hour.
Required Qualifications
- A current PA Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
