Outpatient Physical Therapist, PT, Senior Living Visits

Job Summary
Conduct outpatient home health visits to adult and geriatric clients in Independent and Assisted Living Communities throughout the Hockessin area. Perform diagnostic tests for mobility, range of motion, balance, and strength, then develop and implement individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while collaborating with nursing on multidisciplinary strategies. Educate clients and family members on rehabilitative activities and accurately document all observations and interventions using a state-of-the-art touch pad tablet. Requires a current DE Physical Therapist license, one year of recent experience, and proficiency with electronic medical records. Required Qualifications
- A current DE Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills
