Outpatient Physical Therapist, PT, Senior Living

Job Summary
Make outpatient visits to clients in designated geographic territories within Independent and Assisted Living communities throughout the Uniontown area. Perform diagnostic tests and measurements for mobility, strength, balance, and safety. Develop and implement individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating in multidisciplinary discussions with nursing. Educate clients and family members on rehabilitative care to promote independent living. Accurately document observations and interventions using a state-of-the-art touch pad tablet. Requires a current Ohio Physical Therapist license, one year of recent experience, and graduation from an accredited program.
Required Qualifications
- A current OH Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
