Job Summary
Evaluate assigned patients by gathering data from records, interviews, and observations. Develop and implement individualized treatment programs for patients requiring physical or occupational therapy services. Provide direct therapy using functional settings and strategies tailored to each patient's capabilities. Prepare required documentation including evaluations, treatment plans, and progress notes. Communicate effectively with patients, families, physicians, and interdisciplinary team members regarding status and needs. Establish rapport and collaborate with the team throughout the intervention program.
Required Qualifications
- Active license in the state of practice upon hire
- Ability to work the hours specified
- Visual acuity adequate to perform position duties
- Ability to communicate effectively with others
- Ability to hear, understand and distinguish speech and other sounds
Desired Qualifications
- 1-3 years Experience as a licensed physical therapist
- Lymphedema experience
