Job Summary
Own the full step-down process from patient identification to program admission, meeting every eligible inpatient to review open direct admission inquiries. Educate patients and families on continued care post-discharge, including specialized tracks and barriers like transportation, while maintaining multiple in-person check-ins during the inpatient stay. Partner with clinical services, business office, and utilization review to confirm appropriateness, verify benefits, secure authorizations, and track denial appeals. Facilitate daily flash meetings to review emerging candidates and report weekly progress to leadership. Assist with outpatient orientation, documentation completion, and introductions to PHP/IOP leaders.
Required Qualifications
- High school diploma or equivalent
- Minimum of 2 years in behavioral healthcare setting (e.g., acute psychiatric hospital)
Desired Qualifications
- Bachelor's degree in social services-related field
- Licensed or license-eligible clinician (e.g., LCSW, LPC, LMHC, LMFT)
- Peer support specialist certification
- Experience with PHP and IOP levels of care, discharge planning, and/or utilization review
