Job Summary
Provides ongoing outpatient psychotherapy to patients of all ages and their families in individual and/or group settings, using techniques and interventions individualized to the patient. Conducts initial mental health evaluations to assess need for ongoing psychotherapy or other services. Develops treatment plans with specific goals and objectives to address in psychotherapy. Delivers case management, referrals, and discharge planning; coordinates with significant others, caregivers, employers, schools, welfare agencies, psychiatrists, and other providers with patient permission. Identifies appropriate level of care and facilitates transitions within Behavioral Health; assists with obtaining authorization for services. Maintains timely, concise clinical documentation in the medical record and ensures HIPAA compliance. Maintains patient confidentiality and upholds provider responsibilities per applicable laws and regulations.
Required Qualifications
- Master's degree in psychology, counseling, social work, related field
- Licensed Clinical Social Worker (LCSW) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Licensed Clinical Professional Counselor (LCPC) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Licensed Marriage & Family Therapist (LMFT) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Work Experience: Providing psychotherapy in a clinical setting
- Strong communication and listening skills
- Ability to work effectively with a multitude of individuals at various organizational levels
- Ability to clarify goals and see projects through to completion
- Ability to project a professional, friendly, helpful demeanor
- Provides ongoing psychotherapy to patients of all ages and their families through individual and/or group settings
- Performs initial mental health evaluations for patients, including assessing need for ongoing psychotherapy or other services
- Works with patient to develop treatment plan, including specific goals and objectives to be addressed in psychotherapy
- Provides case management, referrals, and discharge planning
- Coordinates treatment and/or discharge with significant others, caregivers, employers, schools, involved welfare agencies, psychiatrist, and any other care provider as appropriate with permission of patient
- Identifies clinically appropriate level of care; facilitates transition between levels of care within Behavioral Health
- Assists with obtaining authorization for services whenever necessary
- Completes clinical documentation in medical record within guidelines and timeframes established by departmental policy
- Maintains patient confidentiality and fulfills provider responsibilities as specified under HIPAA, the Illinois Mental Health and Developmental Disabilities Code, and the Illinois Mental Health and Developmental Disabilities Confidentiality Act
