Outpatient Licensed Practical Nurse LPN

Job Summary
Direct patient care for dialysis patients as part of the hemodialysis team. Perform technical dialysis procedures, administer treatments per the treatment plan, monitor dialysis parameters, and document patient information in records. Educate patients and families about end-stage renal disease, dialysis therapy, diet, and medications. Initiate basic CPR in emergencies and respond to adverse events. Ensure a clean, safe facility environment; address blood spills per Bloodborne Pathogen policies; troubleshoot equipment issues; collaborate with the Physician, Dietitian, and Social Worker on care plans. Requires graduation from an accredited Practical Nursing program and current LPN licensure; CPR certification; ability to travel between facilities; one year medical-surgical nursing experience preferred; hemodialysis experience preferred but not required.
Required Qualifications
- Graduate of an accredited School of Practical Nursing (LPN/LVN)
- Current state licensure as an LPN
- CPR Certification
- Successfully complete the Nurses Technical Training Program upon hire or soon thereafter
- One year medical-surgical nursing experience preferred
- Hemodialysis experience preferred but not required
- Ability to provide coverage at area facilities during times of short-staffing or as required
