Outpatient Licensed Practical Nurse (LPN) 2 - Children's Multispecialty

Coordinate referrals for patient chronic care conditions and follow-up care based on specialist and primary care physician plans. Administer prescribed oral and intramuscular medications, therapies, and treatments following practical nursing standards under direct physician supervision. Effectively communicate with healthcare team members, patients, and families while supporting team goals and assigning tasks to care assistants. Maintain and review patient medical records, notes, and physician orders; document all patient care activities in accordance with documentation standards and legal requirements.
