Job Summary
Outpatient Case Manager for youth: responsible for the assessment, planning, coordination, and monitoring of Mental Health/Community Services to children ages 0-18 and their families in Western Wayne County. Duties include maintaining monthly face-to-face contact with families in home/school/community settings, developing comprehensive client assessments and service plans, advocating for services, facilitating out-of-home placements when necessary, maintaining a resources database, collaborating with mental health staff, and keeping client files up to date. Requires knowledge of local systems (Community Mental Health, DSS, Juvenile Court, Probate Court, School), flexibility in hours, and reliable transportation. EOE/M/F/D/V.
Required Qualifications
- Must meet criteria as a Child Mental Health Professional (CMHP)
- Minimum of a bachelor’s degree in a mental health, or related field from an accredited school
- Minimum of 3 years of supervised experience in the examination, evaluation, and treatment of minors (ages 0-18) and their families
- Prior experience working in a Community Health setting preferred
