Job Summary
Provide case management to children and families, including identifying and pursuing resources to meet client needs. Deliver in-home or in-office direct services as needed and collaborate with clients and their therapist as part of an interdisciplinary team to alleviate barriers. Assist with securing housing, food, vocational, educational, and supportive services; link clients to other service agencies; attend outside agency meetings; maintain DMH paperwork and deadlines; manage a caseload of 20+ people. Strong DMH/DCFS collaboration and Spanish language skills are preferred.
Required Qualifications
- Bachelor's degree in related field is required
- 2 years experience in social services is required
- Fluency in Spanish strongly preferred
