Job Summary
Responsible for partnering with the physician and multidisciplinary team to coordinate patient care with the objective of enhancing quality and cost-effective care. Acts as a patient advocate to facilitate safe transitions across the continuum. Performs care coordination, completes assessments and plans, monitors length of stay, promotes efficient resource utilization, and ensures seamless continuity of care with the physician as the single consistent resource. Maintains knowledge of clinical practice guidelines, performs problem resolution and effective communication with patients and families, refers appropriate cases for Social Work intervention, and ensures complete documentation in EMR and case-management systems. Requires RN or LSW licensure with 3+ years of relevant clinical experience; salary range $79,719-$123,934 annually; benefits include medical/dental/vision, retirement plans, paid time off, and other wellbeing programs.
Required Qualifications
- RN licensed or Masters prepared Social Worker (LSW) required
- 3+ years clinical nursing or social work experience
- 1+ year Case Management or homecare coordination in acute care or behavioral health setting
- Understanding of Medicare, Medicaid, managed care and discharge planning
- Excellent verbal and written communication, problem solving, critical thinking, conflict resolution
- Documentation in EMR and case management systems
- Ability to engage physician and multidisciplinary team and coordinate care across continuum
