Job Summary
Conduct direct individual and family services to facilitate healing through trauma-informed care and Positive Youth Development for pregnant and parenting adolescent females aged 13-20. Manage a caseload of eight residents by establishing connections to housing, education, employment, and support systems while ensuring regulatory compliance and reporting. Provide 24/7 rotating on-call services and respond in person as needed, while collaborating with the City of Philadelphia Department of Human Services, courts, and community agencies to coordinate intake, discharge planning, and resource linkages. Complete required documentation including Individual Service Plans, quarterly reports, and daily contact logs. Requires a Master's degree in social work or related field, five years of child welfare experience, and the ability to work evening and weekend hours.
