OT - Skilled Nursing Setting

Conduct detailed evaluations of residents' physical, cognitive, perceptual, and psychosocial abilities, then design and implement personalized treatment plans to restore function in activities of daily living and instrumental ADLs. Guide residents through occupation-based interventions, adaptive equipment training, energy conservation techniques, cognitive strategies, environmental modifications, and fall prevention education. Track progress rigorously, document outcomes, educate families and staff, and collaborate with the interdisciplinary team to adjust plans for optimal independence. Lead clinical assessments and program enhancements within a collaborative therapy team while delivering evidence-based care in a stable full-time role.
