Job Summary
Deliver occupational therapy services in patients' homes, completing initial evaluations, follow-up visits, and fall risk assessments. Develop and implement individualized treatment plans, assist with interventions under supervision, and educate patients and caregivers on adaptive techniques. Monitor patient progress, communicate changes with the interdisciplinary team, and maintain accurate clinical documentation. Work independently while collaborating with the team to promote patient independence and safety in the home environment.
Required Qualifications
- Active Hawaii Occupational Therapist license
- Minimum one year of clinical experience
- Active Hawaii Certified Occupational Therapy Assistant license
- Minimum one year of clinical experience preferred
- Strong communication and documentation skills
- Comfortable with community outreach and educational presentations
- Valid driver's license and reliable transportation
- Ability to work independently while collaborating with the interdisciplinary team
Desired Qualifications
- Home health experience preferred but not required
