Job Summary
Conduct initial assessments of patients to understand their physical condition and needs, developing evidence-based rehabilitation plans tailored to individual recovery goals. Implement therapeutic techniques including manual therapy, movement-based treatments, and functional strength training to facilitate optimal recovery. Instruct patients and families on exercises and lifestyle modifications that promote healing and prevent future injuries. Maintain accurate patient records and documentation in compliance with healthcare regulations while collaborating closely with physicians and other healthcare professionals. Provide education and resources to patients on their conditions, supporting their understanding and adherence to prescribed therapies. This full-time role features a standard Monday through Friday schedule from 9 AM to 5 PM in a home care setting within Monroe, New York.
Required Qualifications
- Active license to practice as a Physical Therapist in New York State
- Bachelor's Degree in Physical Therapy
Desired Qualifications
- Master's or Doctorate in Physical Therapy
- At least 2 years of prior experience in a clinical outpatient orthopedic setting
- Knowledge of sports medicine
- Familiarity with rehabilitation tools and equipment applicable to home care settings
- Experience using electronic health record (EHR) systems for documenting patient evaluations and treatment progress
