Job Summary
Direct and implement individualized physical therapy treatment plans to help clients manage pain, improve mobility, and regain functionality. Evaluate clients' physical abilities through comprehensive assessments, supervise physical therapy assistants, and conduct in-depth evaluations of clients' home environments to optimize rehabilitation. Administer therapeutic techniques such as manual exercises and traction while motivating clients through individualized exercise regimens. Participate in interdisciplinary team discussions, review care plans, and complete clinical records ensuring adherence to OASIS standards. Maintain daily communication with the team to adjust treatment approaches as required. This full-time role is onsite at our Carteret, New Jersey facility, serving diverse populations including post-surgical recovery and chronic pain management.
Required Qualifications
- Bachelor's Degree in Physical Therapy from an accredited institution
- Current licensure as a Physical Therapist in good standing within New Jersey
- Proficient understanding of federal, state, and governing agency regulations in physical therapy
- Strong interpersonal communication skills to foster effective patient and family engagement
Desired Qualifications
- A commitment to clinical excellence, quality, and integrity in every aspect of care
- A Doctorate in Physical Therapy
- A minimum of one year of experience working in an acute care or home care setting
- Current CPR certification
- A valid driver's license and automobile insurance for travel to client homes when necessary
- A passion for promoting patient care and advocating for home healthcare services
