Job Summary
Conduct thorough patient evaluations incorporating medical history reviews and targeted assessment protocols. Develop and communicate comprehensive treatment plans tailored to individual needs, then implement evidence-based protocols while adjusting approaches based on progress. Maintain detailed documentation of evaluations, plans of care, and treatment progress in compliance with regulatory requirements. Collaborate with multidisciplinary team members including physicians and nurses to ensure cohesive care and timely referrals. Participate in discharge planning and ensure patients have necessary follow-up recommendations. Engage in professional development by serving as a clinical instructor for student interns and new staff. Contribute to hospital and community outreach initiatives to enhance patient access to care.
Required Qualifications
- A Bachelor's degree in Physical Therapy
- A current state license to practice as a Physical Therapist in New Jersey
- Certification in Basic Life Support (BLS) from the American Heart Association
- Demonstrable commitment to ongoing learning and professional development
Desired Qualifications
- Doctorate in Physical Therapy
- Prior work experience in a similar setting
- Clinical experience in outpatient orthopedic therapy
- Familiarity with electronic health record (EHR) systems
- Familiarity with various physical therapy modalities
