Job Summary
Conduct thorough evaluations of patients by reviewing medical histories and performing specific assessments to formulate effective treatment plans. Implement targeted treatment protocols based on evaluation findings, adapting techniques to align with the latest healthcare standards and individual patient needs. Communicate evaluation findings and treatment plans clearly to patients and family members while engaging them in the goal-setting process. Collaborate with other healthcare professionals to address patient needs, documenting changes in condition and adjusting the plan of care accordingly. Manage discharge planning and follow-up care by maintaining accurate documentation of initial evaluations, progress notes, and discharge summaries. This onsite position entails providing care directly in patients' homes in Union City, New Jersey, adapting treatment plans to their unique living environments.
Required Qualifications
- A Doctorate degree in Physical Therapy from an accredited program
- Current state licensure for physical therapy
- Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association
Desired Qualifications
- Experience in orthopedic rehabilitation or home health settings
- Familiarity with electronic health record (EHR) systems for documenting patient information and treatment plans
- A demonstrated ability to work collaboratively in multidisciplinary teams to achieve comprehensive patient care outcomes
