Job Summary
Conduct thorough evaluations of patients referred by physicians, assessing physical abilities and functional limitations. Design, implement, and adapt individualized treatment plans that reflect the unique needs of each patient using advanced therapeutic modalities and exercise regimens. Engage in direct patient care, demonstrating a hands-on approach to therapy both within the clinic and during home visits. Maintain open lines of communication with patients, providing education and training to ensure continuity of care. Document patient progress and outcomes diligently to meet regulatory requirements. Collaborate effectively with interdisciplinary teams to optimize patient care and achieve the best possible outcomes. This full-time position is primarily onsite with 40-hour shifts Monday through Friday.
Required Qualifications
- A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
- Current and valid State License in Physical Therapy.
- Minimum of two years' experience as a Physical Therapist.
- Basic Life Support Certification from the American Heart Association.
- Excellent communication skills, both verbal and written, to facilitate effective interaction with patients and team members.
- Proficient in using electronic health record systems and other software for patient documentation and management.
- Strong interpersonal skills to foster patient engagement and community relationships.
Desired Qualifications
- Experience working in a multi-disciplinary team environment.
- Specialized training or certification in manual therapy, dry needling, or sports rehabilitation.
