Orthopedic Physical Therapist - Full-Time Home Care Specialist

Job Summary
Conduct comprehensive initial assessments on patients, implementing manual therapy and corrective exercises to address impairment and enhance functional mobility. Perform regular progress evaluations to adjust treatment plans using evidence-based approaches, while educating clients on injury prevention and home exercises. Document patient progress accurately in our electronic health record system. Evaluate and treat individuals recovering from orthopedic surgeries, sports injuries, and chronic conditions within a low-volume, personalized care model. Work onsite Monday through Friday from 9 AM to 5 PM with a small collaborative team.
Required Qualifications
- Valid New York State License to practice as a Physical Therapist
- Bachelor's Degree in Physical Therapy
- Proven ability to conduct thorough patient evaluations and develop effective treatment plans
- Strong interpersonal skills that facilitate communication with patients and healthcare team members
- At least 2 years of professional experience in outpatient rehabilitation or sports therapy
- Familiarity with current rehabilitation technologies and practices, including experience using EHR systems
Desired Qualifications
- Master's or Doctorate in Physical Therapy
- Specialized certifications, such as in manual therapy or sports rehabilitation
