Collaborate with multidisciplinary teams, including physicians and other healthcare professionals, to modify treatment plans as necessary and address any changes in patient conditions. Maintain accurate and timely records of patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations to comply with state and federal regulations. Evaluate treatment outcomes regularly and adjust care strategies to meet evolving patient needs while providing education and support to patients and their families. Engage in ongoing professional development by attending workshops and seminars aimed at enhancing therapy skills and knowledge.
