Job Summary
Evaluate and treat patients recovering from total hip and knee replacements in their home setting, focusing on safe bed mobility, transfers, and ambulation. Collect and record patient care data using Prompt EMR while managing daily billing and productivity guidelines. Formulate diagnoses, goals, and treatment plans based on evaluation results, then collaborate with interdisciplinary teams to facilitate discharge to JAG outpatient locations. Provide education on community reintegration and therapeutic activities, ensuring all interventions comply with department policies.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
