Job Summary
Evaluate and treat patients recovering from total hip and knee replacements in their home setting, focusing on safe bed mobility, transfers, and ambulation. Conduct initial evaluations, administer tests, and synthesize results to formulate diagnoses, goals, and treatment plans. Educate patients and support systems on functional training, including ADLs/IADLs and rehabilitation equipment use. Document all care in the Prompt EMR, manage daily billing, and communicate with the interdisciplinary team and patient access for scheduling. Transition patients toward outpatient care at JAG facilities while maintaining compliance with department policies and reimbursement guidelines.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing in
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Strong orthopedic backgorund
- Masters or better
