Job Summary
Evaluate and treat patients recovering from total hip and knee replacements in their home setting, focusing on safe bed mobility, transfers, and ambulation. Collect and record patient care data using Prompt EMR while formulating diagnoses, goals, and treatment plans based on evaluation results. Collaborate with interdisciplinary teams to establish functional training for ADLs and community reintegration, ensuring all interventions comply with department guidelines. Communicate with the Patient Access team for scheduling and attend annual billing seminars to maintain productivity standards. Transition patients to outpatient locations for continuity of care as they progress.
Required Qualifications
- Bachelor's or Master's Degree
- Licensed Physical Therapist in the state practicing
- Completed state regulated CEU requirement in appropriate state (NY, NJ)
- Valid CPR License
- Experience/knowledge with an EMR software
- Excellent oral, written communication
- Strong clinical decision-making skills
- Excellent work ethic and dedication to patient success
- Basic computer skills
- Works as a team member and individually with minimal supervision
- Maintains a positive, professional demeanor at all times
- Strong orthopedic background
- Masters or better
