Job Summary
Advise veterans and caregivers of available services and resources to support aging in place, utilizing interview skills to screen clients, complete assessments, and develop comprehensive care plans. Provide case management and service coordination for eligible clients in North Carolina and surrounding areas, requiring significant travel with personal vehicle reimbursement at the IRS rate. This full-time, remote role operates Monday through Friday from 8:30 am to 5:00 pm and offers a competitive hourly rate of $24.34. Peninsula Agency on Aging, serving older adults since 1974, values empathy, compassion, and reliability in its team.
Required Qualifications
- Bachelor's Degree from accredited college or university in social work, human services, or related field
- Experience with working with seniors and/or disabled adults
- Case management experience
- Excellent interviewing skills
- Oral communication skills
- Written communication skills
- Effective use of computers, software, and mobile technology
- Ability to model our values
- Vaccination for COVID
- Significant travel utilizing a personal vehicle
- Mileage reimbursement at the IRS mileage reimbursement rate
