Job Summary

Conduct comprehensive HIV/AIDS and specialty care assessments, diagnose acute and chronic conditions, and manage treatment plans including prescribing medications and administering vaccines. Deliver primary care services such as physicals, pap smears, and pelvic exams while reviewing lab results for viral loads and CD4 counts. Coordinate care for newly diagnosed clients, liaise with case management and telehealth providers, and authorize prescription requests based on client needs. Document all encounters in Florida Shots, HMS, and other state systems, submit required reports, and participate in quality improvement activities and training workshops. Work independently in a multidisciplinary team setting with flexible hours including Fridays and emergency shifts.

Required Qualifications

Currently hold an active Florida Advanced Practice Registered Nurse (APRN) license
Verifiable experience as nurse practitioner in a HIV clinical setting
Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency
Willing to travel to different sites in Lake County as needed (Umatilla, Leesburg, Clermont)
All Florida Department of Health positions require the incumbent to be able to learn and communicate effectively, orally and in writing, in English

This position requires a security background check and/or drug screening
Successful completion of a drug test is a condition of employment for safety-sensitive positions
The incumbent must be able to interview clients about HIV/AIDS and high-risk activities
The incumbent must be comfortable interviewing clients about HIV/AIDS and high-risk activities
The incumbent must be able to conduct thorough assessments, diagnose and manage both acute and chronic conditions
The incumbent must be able to deliver evidence‐based treatment with a focus on prevention, wellness, and patient education
The incumbent must be able to diagnose and treat various secondary and opportunistic infections like sexually transmitted infections, hepatitis, and tuberculosis
The incumbent must be able to review lab results and provide follow‐up on abnormal findings—especially viral loads and CD4 counts
The incumbent must be able to assess medication adherence, identify barriers such as missed appointments or issues with housing and transportation, and implement needed clinical recommendations
The incumbent must be able to review and authorize prescription requests based on client needs
The incumbent must be able to evaluate immunization status, administer vaccines per protocol, and document all immunizations and clinical services
The incumbent must be able to enter medication refill requests into HMS–Emdeon
The incumbent must be able to enter all client encounters—whether face‐to‐face, via telehealth, or by phone—into the required systems for tracking and reporting
The incumbent must be able to submit reports, maintain records, and complete paperwork—including timesheets, mileage, and EARS—accurately, legibly, and on time
The incumbent must be able to answer calls promptly, speak clearly and professionally, and assist with clerical functions when needed
The incumbent must be able to stay current with HRSA policies and procedures
The incumbent must be able to assist with chart reviews, engage in QA/QI activities to support DOH-Lake's mission, and participate in the Quality Advisory Committee
The incumbent must be able to take part in case discussions, case conferences, ADAP and DOH‐Lake meetings, and collaborations with community partners
The incumbent must be able to transcribe and distribute meeting minutes within two days
The incumbent must be able to report any client concerns immediately to the Manager or designee
The incumbent must be able to maintain confidentiality and the security of all records as required by departmental regulations and laws
The incumbent must be able to work independently, plan, organize and coordinate work assignments
The incumbent must be able to communicate effectively, establish and maintain effective working relationships with others
The incumbent must be able to utilize problem solving techniques and use personal computers
The incumbent must be able to deal with the public in a tactful and courteous manner
