Job Summary
Operations Coordinator supports patient intake, arranges care logistics, and facilitates communication across clinical and administrative groups in the Home Hospital model. You will screen, obtain consent, and enroll patients, monitor admissions and referrals, manage supplies and equipment, and uphold critical workflows to deliver quality care at home. The role involves on-site work with occasional face-to-face meetings with patients and families, and telephonic outreach as part of care coordination. Works collaboratively with nurses, providers, and the Business Operations Manager; 24 hours per week scheduled Wednesday–Friday, 12:00pm–8:00pm at Salem Hospital.
Required Qualifications
- High School Diploma or Equivalent required
- Associate's Degree Related Field of Study preferred
- 2-3 years required
