Job Summary
Conduct comprehensive examinations and evaluations of patients to determine functional limitations, then develop tailored treatment plans to enhance mobility and physical functioning. Provide ongoing consultation to healthcare providers and families regarding the continuum of care, while documenting evaluations and treatment plans meticulously to ensure compliance with laws and regulations. Implement treatment procedures, re-evaluate patient progress regularly, and educate patients and caregivers on practical recovery strategies. Maintain open communication with team members, referral sources, and physicians to ensure integrated treatment plans are in place. Participate in leadership functions including community outreach, marketing initiatives, and continuous quality improvement projects, while providing clinical education and supervision to less experienced staff members and students.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited program
- Active State License in Physical Therapy
- Basic Life Support (BLS) certification
- Proficiency in written and verbal communication skills
- Computer skills for documentation and communications
- Ability to demonstrate a proactive, patient-focused approach
- Onsite presence in East Hartford, Connecticut
- Full-time employment
- Onsite physical presence for home care services
