Job Summary
Assess and evaluate patients' conditions to develop customized treatment plans aimed at improving mobility and reducing pain. Implement therapeutic techniques including exercises, stretching, hands-on therapy, and specialized equipment based on established protocols. Carefully track and document treatment responses in patient records while regularly reviewing and modifying plans to ensure goals are met. Assist patients in obtaining and fitting assistive devices, providing education to enhance independence. Train patients, families, and caregivers on therapy techniques and equipment usage. Maintain professional knowledge through workshops, seminars, and networking opportunities. Work onsite in a home care setting with diverse pediatric to geriatric populations, collaborating closely with the team to foster patient wellness and satisfaction.
Required Qualifications
- A Doctorate in Physical Therapy (DPT)
- A valid DPT license in the State of New Jersey
- Demonstrated excellent written and verbal communication skills
- Proficient in using standard computer applications and electronic health record (EHR) systems
- Flexibility and adaptability in a constantly changing home care environment
Desired Qualifications
- Minimum of 2 years of experience in a clinical setting, preferably with home care or outpatient services
- Experience with diverse patient populations and conditions
- Prior exposure to assistive technology and rehabilitation equipment
