Oncology Nurse Navigator

Guide patients through the healthcare system from diagnosis through survivorship by coordinating care, providing education, and eliminating logistical and financial barriers. Act as a liaison between patients, families, and the medical team, managing all referrals for Oncology and Hematology, including those for external care. Sit in with providers on new and follow-up visits as needed, oversee new patient appointments, and conduct new patient chemotherapy teaching. Delegate appropriate duties to trained staff when higher-scope tasks require priority. Coordinate time off with a backup navigator to ensure adequate coverage on all business days.
