Job Summary
Conduct comprehensive clinical assessments and support members throughout the oncology care continuum via phone, email, and video communication. Perform a minimum of 20 member calls daily while coordinating care, managing symptom relief, and facilitating end-of-life discussions. Collaborate with Nurse leaders and Medical Directors to ensure alignment with clinical protocols, participate in case conferences, and address social determinants of health needs. Identify and address member needs promptly, offering assistance with care coordination, nutritional support, and provider referrals. Be available for urgent clinical escalations and provide clinical consult support as required. Work a fixed schedule of 8:30 AM to 5:00 PM EST, four weekdays and Saturdays, with the designated weekday off being Monday, Thursday, or Friday.
Required Qualifications
- An active, unrestricted compact Registered Nurse (RN) license
- Degree in Nursing (ADN or BSN)
- At least 5 years of nursing experience
- 3 years in solid tumor oncology nursing
- Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM)
- Located within the lower 48 United States
Desired Qualifications
- BSN
- Compact + single state licensure in New York and/or Illinois licensure
- Fluency in Spanish, both written and verbal
- Oncology-related certifications obtained within 2 years of hire
