Job Summary
Assess, plan, and coordinate care to meet patients' physical, emotional, and social needs while advocating for timely, high-quality care through the healthcare system. Coordinate appointments, referrals, and services to ensure continuous care, providing education on treatment plans, symptom management, and health maintenance. Maintain accurate documentation, reduce barriers to care via insurance coordination and community resource navigation, and collaborate with oncologists and support teams to enhance patient outcomes. Perform RN scope duties including telephone triage and clinical assessments, and participate in clinic activities. No nights, weekends, or holidays.
Required Qualifications
- Graduate of an accredited nursing program
- At least 1 year of nursing experience
- Current RN license in the state of practice or compact license
- BLS certification through the American Heart Association
- Strong knowledge of oncology practices and patient navigation
- Excellent verbal and written communication skills
- Critical thinking and conflict resolution skills
- Ability to prioritize, manage time effectively, and work both independently and collaboratively
- Proficiency with electronic documentation and EMR systems
- Compassionate, patient-centered approach with strong attention to detail
Desired Qualifications
- Bachelor's degree in nursing
- Outpatient nursing and scheduling experience
- OCN certification
