Job Summary
On Call RN provides intermittent skilled nursing care to patients in their place of residence, following physician orders and the plan of care under the supervision of the Clinical Manager. Responsibilities include performing the initial evaluation visit, assessing needs and eligibility, developing and revising the plan of care with the interdisciplinary team (IDT), initiating and supervising nursing procedures, preparing progress notes and summaries, coordinating services with physicians and other providers, counseling patients and families, participating in in-service programs, supervising LPN/LVN and HHA/CNA per regulatory requirements, completing OASIS assessments and related documentation, and taking on-call duties nights, weekends, and holidays. Requires graduation from an approved nursing program, current licensure, and CPR certification; one year of nursing experience is preferred.
Required Qualifications
- Graduate of an approved school of professional nursing
- Current nursing license in the practicing state(s)
- One (1) year nursing experience, preferred
- Ability to exercise initiative and independent judgment
- Acceptance of philosophy and goals of the Agency
- Provides services in accordance with the plan of care
- Performs initial evaluation visit and develops plan of care in coordination with IDT
- Coordinates services with physicians and care providers
- Supervises LPN/LVN and HHA/CNA as per regulatory requirements
- Maintains documentation and reports to administrator/clinical manager
- Maintains CPR certification
- Maintains confidentiality and adheres to agency policies
