Job Summary
Provide compassionate care to patients and families in home hospice and palliative settings. Manage daily patient needs while coordinating with the home health team. Maintain accurate documentation and adhere to strict clinical protocols. Requires a valid Wisconsin RN license, current driver's license, and at least one year of home hospice or home health experience. Full-time role includes a 7 on/7 off schedule with day and evening shifts from 4PM to 8AM Monday through Sunday. Company provides a vehicle and covers all travel expenses.
