Job Summary
Provide exceptional hospice care to patients and families by visiting homes, assisted living facilities, and hospitals for symptom management, admissions, and deaths. Handle hospice admissions, answer calls from patients and families, and manage nursing visits as needed. Prioritize and triage urgent concerns while documenting visits and communicating with the patient care team. Work independently after hours on a seven-on, seven-off on-call rotation. Requires current RN licensure, one year of experience, and reliable transportation to drive across the service area.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance, to be able to travel across the service area
- Must be able to work after hours on-call seven days/nights on and seven nights/days off
