Job Summary
Provide care and services for hospice patients by visiting them in private homes, assisted living facilities, and hospitals. Complete hospice admissions and patient visits as needed, including symptom management, deaths, and admissions. Answer calls from patients, families, and customers while managing time to prioritize and triage multiple urgent concerns. Work independently to handle phone, email, and written communication, and learn the electronic medical records system. This role requires current RN licensure with one year of experience and involves after-hours on-call shifts with a seven days on, seven days off schedule.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
- Must be willing to drive with reliable transportation, valid driver's license, and auto insurance, to be able to travel across the service area
- Must be available for after hours on-call working seven days/nights on and seven nights/days off
