Job Summary
On Call Registered Nurse for Luminary Hospice. Provides after-hours nursing care in patients' homes and communities, with evening shifts Monday–Thursday (5:00 PM–8:30 AM) and a continuous weekend shift Friday 5:00 PM–Monday 8:30 AM, on a rotating schedule (one week on, one week off). Performs comprehensive patient and family assessments, develops and revises plans of care, administers prescribed medications and treatments, assists with activities of daily living, and coordinates post–hours patient care with Case Managers. Delivers patient and family education, supports emotional needs with the interdisciplinary hospice team, documents clinical notes and care outcomes, communicates with physicians, and participates in quality improvement activities. Requires a nursing degree, an active state license, CPR certification, and relevant hospice/palliative care experience or training.
Required Qualifications
- Bachelor's or Associate's degree in nursing or related fields
- Active nursing license in good standing in the state performing job duties
- Active CPR certification
- 2-5 years of experience in healthcare management, hospice, or palliative care and/or training in end-of-life care (preferred)
- 1 year of experience with Medicare Hospice Certification and CHAP guidelines (preferred)
- Knowledge of hospice care and interdisciplinary team services
- Proficient in MS Office and ability to learn job-specific software
- Strong organizational, communication, analytical, and attention-to-detail skills
