Job Summary
Assess patient and family situations for severity, respond appropriately, and provide holistic end-of-life care in homes, units, and community facilities. Evaluate physical, financial, psychological, and spiritual needs to determine hospice eligibility, coordinate medication administration, and update care plans as conditions change. Educate caregivers on disease processes, emergency responses, and treatment regimens while promoting independence and providing emotional support. Make visits as indicated, assist with bereavement follow-up, and communicate regularly with interdisciplinary teams and physicians. Available for on-call rotation with 15-minute response times, requiring current NC licensure and two to three years of nursing experience.
Required Qualifications
- Current NC license as a registered nurse
- Active patient contact or completion of a nursing refresher course within the past three years
- Minimum of two to three years nursing experience
- CPR certification
- Ability to work a flexible schedule
- Willingness to work with patients/families regardless of race, color, creed, age or sexual orientation
- Ability to work effectively with others in a supervisory role
- Strong communication skills
- Visual acuity to read written documentation and computer tasks
- Ability to sit extended periods of time and perform administrative tasks
- Ability to stand, bend and lift an average of 30 pounds
- Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time
- Ability to walk, squat, kneel, climb and balance
