Job Summary

Assess patient and family situations for severity, respond appropriately, and provide holistic end-of-life care in homes, patient care units, and community facilities. Coordinate medication administration, update plans of care based on changing conditions, and educate caregivers on disease trajectory, emergency responses, and treatment regimens. Determine hospice eligibility with AMOREM providers, refer to community agencies when appropriate, and support individual coping mechanisms while promoting patient independence. Respond to communications within 15 minutes, make visits as indicated, and assist with bereavement follow-up including receiving or funeral services. Requires current NC license, two to three years of nursing experience, CPR certification, and ability to work a flexible schedule.

Required Qualifications

  • Current NC license as a registered nurse
  • Active patient contact or completion of a nursing refresher course within the past three years
  • Minimum of two to three years nursing experience
  • CPR certification
  • Ability to work a flexible schedule
  • Willingness to work with patients/families regardless of race, color, creed, age or sexual orientation
  • Ability to work effectively with others in a supervisory role
  • Strong communication skills
  • Visual acuity to read written documentation and computer tasks
  • Ability to sit extended periods of time and perform administrative tasks
  • Ability to stand, bend and lift an average of 30 pounds
  • Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time
  • Ability to walk, squat, kneel, climb and balance
