On-Call Registered Nurse (7 On 7 Off) Hospice - Houston, TX

Provide skilled nursing care, including medication administration and treatments, for hospice patients in home, assisted living, and skilled nursing facility settings. Perform comprehensive assessments, coordinate care plans with caregivers, and observe patient status to report changes to supervisors and physicians. Deliver triage, visits, and pronouncements during the 5pm to 8:30am schedule with a 7-on-7-off rotation. Maintain clinical documentation in compliance with agency standards and participate in weekly interdisciplinary team meetings. Requires RN licensure, 1-2 years of experience, and previous hospice care.
