On-Call Physical Therapist

Evaluate residents, conduct ongoing assessments, and plan therapy interventions. Develop and revise care plans based on resident needs, establish functional goals and outcomes, and create discharge plans for long-term and short-term residents. Educate the health care team, residents, family, and caregivers regarding the care plan. Requires current state licensure in Physical Therapy in good standing and a Bachelor's degree.
