On-Call Nurse

Job Summary
Provide after-hours hospice nursing care in patients' homes and communities; perform comprehensive patient and family assessments; develop and revise care plans with palliative nursing actions; administer medications and treatments; assist with activities of daily living; coordinate with Case Managers; document clinical notes; communicate with physicians; educate patients and families; participate in quality improvement; require Bachelor's or Associate's degree in nursing, active license, CPR, 2-5 years healthcare/hospice experience; MS Office proficiency; strong organizational, verbal/written communication, attention to detail; physical ability to lift up to 50 pounds.
Required Qualifications
- Bachelor's or Associate's degree in nursing or related fields, required
- Maintains an active nursing license that is in good standing in the state performing job duties
- Maintains an active CPR certification
- At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred
- At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred
- Knowledge of hospice care and the services provided to patient and family/caregiver through an interdisciplinary group
- Proficient in MS Office applications and ability to learn department and job-specific software systems
- Demonstrate organizational skills
- Demonstrate effective verbal and written communication skills
- Demonstrate analytical skills when problem-solving
- Demonstrate high attention to detail and a high degree of accuracy
